To participate, businesses must provide basic credentials so we can set up your account correctly.
Required Documentation & Setup Steps
1. Business Documentation
To set up a wholesale account, you will need to submit the following:
Resale/Tax ID Documentation (Required)
To purchase tax-free (no sales tax), you must provide a Resale Certificate or Sales Tax Permit/Resale Number from your state. This shows purchases are for resale, not personal use (resale and wholesale permits are commonly required for tax exemption).
IRS Form W-9 (Required)
We require a completed W-9 form from all wholesale buyers. This gives us your business name, taxpayer identification number (EIN), and certification — which helps us maintain accurate tax and vendor records.
Dunn & Bradstreet Number (Required)
We require a DUNS number from all wholesale buyers. This gives us a credit reference to your business.
These documents ensure you’re compliant with tax laws and help us process your account quickly.

Use your own FedEx, UPS, DHL, USPS, or freight account — and we’ll ship your order on your terms.
QuickChange Shipping
Don’t have an account? You may choose to have us ship using our carrier options — we’ll invoice you accordingly.
Alternative Purchasing Options
- If you already work with a Medline representative, you may order through them as well.
- However, any business eligible for our wholesale program is welcome to order directly through us, regardless of Medline affiliation.
Tips for Quick Approval & Success
- Submit accurate resale documentation up front — this avoids sales tax and speeds setup.
- Label your documents clearly with your business name and address.
- Keep copies of your W-9 and resale certificate on file for renewing or re-verification needs.
- If you don’t yet have a resale permit or EIN, contact your state’s Department of Revenue or the IRS to apply — having these ready makes the process smoother.
